Eyelid surgery sits on a line that confuses almost everyone. The same operation can be cosmetic for one patient and medically necessary for another, and insurers draw that line with specific criteria rather than opinions.
Functional versus cosmetic
Functional surgery addresses a lid that interferes with vision or eye health. Cosmetic surgery addresses appearance. Many patients have both concerns at once, which is normal and does not disqualify you, but coverage decisions rest on the functional part.
- Ptosis is drooping of the upper lid itself, often from stretching of the muscle that lifts it. See our ptosis repair page.
- Dermatochalasis is excess upper lid skin that hangs over the lid margin and can block the upper field of vision.
- Entropion and ectropion are lids that turn inward or outward, causing irritation or exposure.
What insurers typically require
- A visual field test with and without the lids taped up, showing measurable improvement when the lid is lifted
- Photographs documenting lid position
- Measurements of lid height relative to the pupil
- A clinical note describing symptoms, such as difficulty driving, reading, or bumping into the upper field
Requirements vary by plan, and Medicare and commercial insurers publish their own thresholds. Testing is done in the office and then submitted for authorization before surgery.
Symptoms worth mentioning at your visit
- Lifting your brows or tilting your head back to see
- Eyelids feeling heavy by the end of the day
- Losing the top of your field of vision while driving
- Repeatedly pushing the lid up with a finger to read
What recovery looks like
Eyelid surgery is usually an outpatient procedure with local anesthesia. Bruising and swelling are expected for one to two weeks, cold compresses help, and most patients are back to normal routines quickly. Full settling of the final appearance takes longer.
Frequently asked questions
What if I want both the medical and cosmetic parts done?
That is common. The covered portion is billed to insurance and the elective portion is quoted separately, so you know both numbers before scheduling. Our cost and insurance page explains it.
Will surgery fix under-eye bags too?
Lower lid surgery is a separate procedure and is usually cosmetic. It is not the same operation as upper lid ptosis repair.
How long does authorization take?
Often a few weeks, depending on the plan. Building in time is wise, especially near year end.
